You can download the passport application
forms you need and guidance on how to complete them from this page.
The forms are in Portable Document Format (PDF) and to read them
you will need to have the Adobe Acrobat Reader program installed
on your computer. This is a free program. You can download it below
if you do not already have it.

embassy with the relevant required documentation in order that their
